Eligibility Requirement
The individual must:
3 or more years (cumulative or consecutive) in an inpatient psychiatric hospital during the 5 years prior to referral.
Emergency Room or Urgent Care
Emergency Room or Urgent Care
Eligibility Requirement
The individual must:
15 or more emergency rooms / Urgent Care visits within 3 years / and 2 psychiatric visits within 3 years.

4 or more Arrests / and 2 crises within the past 3 years.
